Skylights are treated as holes under §1910.28 — an unguarded or non-load-rated skylight is a fall-through fatality waiting to happen, and roof hatches left open are the same hazard. Workers routinely fall through skylights they assumed would hold their weight; they won't.

OSHA cites unguarded skylights and hatches on inspection and it's a known fatality category, so it draws NEP attention. The duty is continuous — guarding has to be in place whenever the roof is accessed.
